Surgical Date Reservation & Deposit Policy 

 

Deposit Requirement & Payment Balance

 

A non-refundable $1,000 deposit is required at the time of scheduling to reserve your surgical date, surgeon time, and operating facility access. This deposit is applied directly to the total cost of your procedure. The remaining balance must be paid in full at least 14 calendar days prior to the scheduled surgery date at the time of your Pre Operative appointment.

 

Rescheduling Schedule

30+ Days Prior to Surgery: You may reschedule your procedure once with no penalty. The full $1,000 deposit will transfer to the new date (must be within 6 months).

 

14 to 29 Days Prior: A $250 administrative re-booking fee applies. The remaining $750 deposit carries over to the new date.

 

Fewer than 14 Days Prior: Rescheduling within 14 days results in forfeiture of the initial deposit. A new $1,000 deposit will be required to secure a future date.

 

Cancellation & Refund Terms

30+ Days Prior to Surgery: You may Cancel with no penalty. The full $1,000 deposit will be refunded.

14-29 Days Notice: Cancellation yields a partial refund of $750 ($250 retained for administrative processing).

 

Less than 14 Days Notice: The $1,000 deposit is strictly non-refundable.

 

Medical Exceptions

 

If your surgeon or anesthesiologist deems you medically unfit for surgery during pre-operative screening, or if an unexpected medical condition arises (documented by a physician note), 100% of your deposit will be refunded or applied toward a future date without penalty.

 

Accepted forms of payment :

 

Credit card, Zelle, Cashier's Check, Cash. Personal check.

Financing programs e.g Cherry, Care credit are not accepted.
